Please sign in using your last (family) name and the ID number listed at the top of the Annual Meeting Registration Confirmation form. The system will then ask you to indicate the sessions you attended, to complete evaluation forms for each of those sessions, and will generate a PDF certificate which may be printed or saved to your computer.
